How would you like to pay ?
Once your order is received and reviewed, you will receive an invoice via email for your deposit. Your reservation will not be guaranteed until deposit is received. Final payment must be received within 30 days prior to event.
After the form is completed, we will send you and invoice from square to pay the $200 retainer for your rental.

If event is located outside of the Treasure Valley, there will be an additional travel fee.
The photo booth requires power to operate. We can plug in to a standard wall outlet or supply a generator for outdoor events if needed.
1 hour to set up is included in the rental. If you need the booth set up earlier, and it will be idle prior to booth start time, there is an additional fee.

If this is a school event, charity fund raiser or other non profit you will be asked to upload your ST-101

Price listed is an estimate. Final price of rental may change based on modifications.
Package includes: Unlimited photos for the length of the event, your choice of Black and White or Color images, custom names and dates on prints (predetermined before event), one photo booth attendant for set up and tear down, and online gallery with all of the original high-resolution photos. Please provide one 6-foot table.
Payment: A $200 deposit is required to reserve the photo booth for your event. Remaining balance is due 2 weeks prior to event. Make checks payable to Photo Booth Boise (all payments are due and payable in Boise, ID).
An additional hour ($100) can be determined at the event.
You will receive an invoice from Photo Booth Boise after completion of this form.

DO NOT AGREE TO THIS CONTRACT UNTIL YOU HAVE READ All THE TERMS. WE ACCEPT THE TERMS OF THIS CONTRACT AND WE ACKNOWLEDGE RECEIPT OF A FULLY COMPLETED COPY OF THE CONTRACT, AND WE ACKNOWLEDGE THAT THE DEPOSIT/RETAINER OF $200 IS NON-REFUNDABLE AND REQUIRED TO RESERVE LAUREN T KISTNER ARTS SERVICES ON YOUR DATE. IF PAYING BY CHECK, MAIL THIS FORM AND RETAINER BACK TO PHOTO BOOTH BOISE / BOISE PHOTO BOOTH AND KEEP ONE COPY FOR YOURSELF. CALL WITH QUESTIONS.
